OCRA-CHIP Grant: Community Health Improvement Plan
The State Educational Institution implemented a demonstration project to build and leverage strong community networks in Decatur and Daviess Counties. The State Educational Institution used these community networks to coalesce a broad group of stakeholders to tackle community and health challenges in a holistic manner and to develop preparedness for future rural health crises. The State Educational Institution, in cooperation with Purdue Extension and Indiana Rural Health Association, developed two (2) county health profiles about needs related to COVID-19 among key leaders and community residents by September 30, 2020, created one (1) community health improvement plan (“CHIP”) responding to immediate and emerging needs related to COVID-19 by January 31, 2021, adapted one (1) CHIP responding to immediate and emerging needs related to COVID-19 by January 31, 2021, and developed one (1) practice brief that highlighted initial outcomes of communities' response to needs related to COVID-19 by May 31, 2021.
